As promised to the community at the time of the launch in September 2013, the Hospital at Home team in Prince Edward County would like to invite you to follow-up press release to hear the results of the program to date. It is indeed generating some promising results in terms of the Ministry of Health’s mandate of improving health outcomes, patient-centredness, and reducing costs.
