Home and Community SELHIN - Care Coordination

The Home and Community South East LHIN is one
of 14 Access Centres established by the Ministry of Health and Long Term Care
to provide communities with access to government funded home and community
services and access to long-term care home. Working closely with the
Prince Edward Family Health Team, a SELHIN case manager helps patients
and their families to navigate through the process of accessing necessary
health services.

Goals

The two main goals of the Home and Community South East LHIN are:

• To be a gateway for information and high-quality health services
• To improve access to health services across the health care continuum
